Each Mental Health Professional at LifeSquared Counseling & Consulting is an independent private practitioner and each therapist DIFFERS in the way finances are collected.

Payment is always due at the time services are rendered. Payment arrangements are outlined below.

    with Mental Health visits

…A SPECIAL NOTE TO EMPLOYEED PEOPLE SEEKING MENTAL HEALTH SERVICES:

  • Your visit with a licensed mental health professional is considered a “Medical appointment” when it comes to communicating with the world you live in and work in. you never need to tell anyone anything other than “I have a medical appointment”

  • Employers have been placed “ON NOTICE” from a federal level to do all possible to ensure each employee has access to and attends all medical appointments

  • Employers are required to make “reasonable accommodations” during the business day for each employee to attend to their personal medical appointment needs

  • IF YOUR EMPLOYER DENIES YOU THE ABILITY TO ATTEND A SCHEDULED MEDICAL APPOINTMENT YOU NEED TO CONTACT YOUR hr DEPARTMENT IMMEDIATELY SO THEY MAY REFRESH THE EMPLOYER ON LEGAL RISK OF FEDERAL LAW VIOLATION(S)


**Out Of Network Provider:

If your insurance is not listed above, we are considered an “Out Of Network” provider.  Your insurance provider may or may not provide you with benefits when you attempt to see Out Of Network providers. You must call and check with your insurance. Grab your insurance card and look for your Member Services phone number.

Private Pay Cash Options:

Many clients either have no insurance or wish to limit the exposure of their mental health services to employers by paying cash for each session.  Many mental health professionals do not accept insurances as it is difficult system to navigate.  We gladly accept all major credit cards for services rendered.  We are affordable with 45-55 minute individual / couple / family sessions starting at $95.00+. (Each Therapist sets their own rates for their private practice clients.)

Low Cost/No Cost Services:

This option requires you to email us to obtain our request form. Not all Low Cost/ No Cost requests can be approved. We do not believe in "Sliding Scale" services.  It violates your privacy by requiring you to provide to us highly personal records of your income and tax return information. Instead, we maintain an allotment of appointments that we offer at low cost / no cost for those struggling financially, or find themselves in a temporary financial hardship.  Please use the link at the top of this page to email us with your request as these appointments are limited.  We will gladly email you our form to fill out and email back to us.
